Athletic and Activity Eligibility Forms
The following forms are used by the Sumner School District Athletic
Department for participation in extra-curricular athletic activities.
Athletic Forms
Transportation for athletic events
The Sumner School District will not provide transportation for student
athletes in the following situations:
1. Off-campus practice sites
2. High school, Varsity and Junior Varsity game sites within the Sumner
School District
3. Return trips from away sites within the Sumner School District for
high school Frosh/”C” team games, and middle school contests
The district will provide one-way transportation to an away site within
the Sumner School District for high school Frosh/”C” team
games and middle school contests, and will provide round trip transportation
to away sites outside the Sumner School District.
