The following forms are for the current school year only and are available for your convenience. All forms must be printed, completed, and returned to the school or District office.
New Student Enrollment Registration Packets
As required by RCW 28A.225.290, the Office of the Superintendent of Public Instruction (OSPI) publishes Learning by Choice every year. This guide outlines the various enrollment options for parents/guardians and students. The guide is available on the OSPI website.
Waiver Applications for the 2016-17 school year need to be submitted January 1 - April 15, 2016, for optimal consideration.
Waiver Process Overview and Timeline
Resident students who wish to attend a school other than their resident school must complete the In-District Waiver Application. In-District Waiver Applications are not required each year for continued attendance unless transitioning to another school. (Example: Elementary to Middle School, or Middle School to High School)
In-District Waiver Application
For students residing outside of the Sumner School District Attendance Boundaries, an Out-of-District Waiver Application is required. A Choice Transfer Request (Release of Attendance form) from the resident district is also required with the Out-of-District Waiver Application. Out-of-District Waiver Applications are required each year for continued attendance.

For students wanting to transfer out of the Sumner School District, a Choice Transfer Request must be submitted to District Administration.
